About this list

The Alabama Medicaid Agency, Program Integrity Division, publishes this Provider Sanction List from its Suspended Providers page. The state uses all three words. The page heading is Suspended Providers, the file is named Provider Sanction List, and the workbook title says individuals excluded from the Alabama Title XIX Medicaid program. At the September 2026 verification the workbook held name-and-date rows for individuals and organizations, grouped under provider-type banners, with no national provider identifier, date of birth, or license number on the row.

Each entry carries a single name cell (usually last name, then first name, sometimes with credentials, aliases, or an owner clause), a suspension effective date, and who initiated the suspension (Medicaid, Medicare, the Medical Licensure Commission, or another named board). A candidate match here rests on name comparison and needs verification against the state. The initiator is stored in the state's own words.

The workbook at verification said newly added sanctioned providers are highlighted and that the list is updated monthly. There is no reinstatement or end-date column. A party leaves the list only by being dropped from a later publication. A clean result against this list is not a finding that no Alabama suspension exists.
