Exclusion screening for healthcare employers

Monthly exclusion screening, with an audit PDF for every run

Upload your roster once. Every month it is screened against the OIG LEIE, SAM.gov, and 43 state exclusion, debarment, and termination lists, and you get a timestamped audit PDF that records which source versions were checked and how each potential match was resolved. When a surveyor asks how you screen, you hand them the report.

Built for the organizations that have to prove monthly screening

Home health and hospice agencies, behavioral health and ABA providers, dental and physician groups, FQHCs, pharmacies, and the billing companies and compliance consultants who support them. Anyone who bills Medicare or Medicaid and has to answer "how do you screen?" with more than a folder of screenshots.

Screening by hand

  • 45 separate sources, each searched one name at a time
  • Repeated every month, for every employee, contractor and vendor
  • Evidence is whatever screenshot someone remembered to save
  • No record of which version of a list was searched, or when it last changed

With ExclusionSentry

  • One roster upload, screened against every source on every run
  • Runs automatically each month on the day you choose, plus on demand
  • An audit PDF per run citing the exact source versions and checksums
  • Every potential match kept, shown with evidence, and its resolution recorded

How it works

1 · Upload

You upload a CSV of names; column mapping and row-level validation are built in. Uploads containing SSNs are refused. Matching uses name, date of birth, and NPI, and license numbers stay on your roster for review records.

2 · Screen

Every name is checked against archived, checksummed copies of the federal and state exclusion data. Potential matches are shown with side-by-side evidence, and the software never suppresses or auto-clears any of them.

3 · Report

Each run produces an audit PDF that cites the exact source versions used and records its own SHA-256 so the file can be verified later. If any source was not current, the PDF says so on page one. See a sample report (PDF).

What the audit PDF contains

When a surveyor or payer auditor asks how you screen, you can hand them the report from any run. Every run produces a PDF that records:

  • The exact source file versions checked, with their checksums.
  • A page-one notice if any source was not current at screening time.
  • Each potential match and how it was resolved, with the evidence shown side by side.
  • The report's own SHA-256, recorded so the file can be verified later.

How the screening is built

Matches are never auto-cleared

The software never suppresses or auto-clears a candidate match, and every resolution is recorded in the audit PDF.

No SSNs

Uploads containing SSNs are refused. Matching uses name, date of birth, and NPI.

Freshness is public

The status board shows the freshness of every source and updates continuously. If a source is stale, the status board and the audit PDF both say so.

Self-serve

You can sign up and run your first screening the same day, with no sales call. Pricing is public.
