About this list
The OMIG list names both individuals and businesses excluded from New York Medicaid, mixed together in a single provider name column with no field marking which is which. Roughly 400 rows also carry an also-known-as or doing-business-as name written inline with the primary name. At the July 2026 verification the file held 8,975 rows. The list reflects current exclusions: there is no reinstatement column, and a party who is reinstated simply leaves the file.
Each row publishes five fields: provider name, license number, NPI, provider type, and the exclusion effective date. Names appear surname first with no comma. The file contains no date of birth and no address. An NPI is present on about 26 percent of rows and a license number on about 67 percent. Because the list has no DOBs, candidate matches from it rest on name comparison, supported by a license number or NPI where one is present, and need more verification from the reviewer than a source that publishes dates of birth.
OMIG publishes the list as a tab-delimited text file, the format this page links; spreadsheet and web versions of the same data also exist. Removals are visible only by comparison: a reinstated party's row is present in one day's file and absent from the next, and no field within the file records that the removal happened.
