About this list
The Texas file is a cumulative history. Rows stay in the file after an exclusion ends, marked with a reinstated date. At the July 2026 verification the file held about 13,384 rows, and about 1,454 of those carried a reinstated date, meaning those parties are no longer excluded. The file also records an eligible-to-reapply date. That is a separate field: only the reinstated date records that an exclusion has ended.
Each row carries 13 columns, including an occupation field, the reinstated date, and the eligible-to-reapply date. The file publishes no date of birth, and an NPI appears on only about 4 percent of rows. In practice most candidate matches from this list rest on name comparison, so a reviewer should expect them to need more supporting verification before reaching any conclusion.
The data comes from the HHSC Office of Inspector General's exclusions search portal, which this page links, and downloads as a dated tab-delimited text file. Reinstatements are recorded directly in the data: a reinstated party's row remains in the file with the reinstated date filled in, so the end of an exclusion is visible in the file itself.
