About this list
This is the exclusion list the Arizona Health Care Cost Containment System publishes through its Office of Inspector General, naming parties excluded from the state's Medicaid program under Arizona Revised Statutes section 36-2930.05. At the August 2026 verification it held 715 distinct entries, naming both individuals and organizations. Arizona publishes a separate list of provider suspensions and terminations that is not part of the file this page describes.
Every entry carries a provider name and an exclusion effective date. The identifier column holds a national provider identifier on about half the entries; on the rest the state either records that it identified the party by another means without publishing the number, or records no identifier at all. No date of birth and no license number are published, so a candidate match here rests on name comparison supported by an NPI where one is present, and needs more verification from the reviewer.
Entries also carry a scheduled end date for the exclusion term, and a few are marked permanent. That scheduled date is not a reinstatement: parties whose end date has already passed remain on the published list, so we never treat it as a return to good standing. Removal from a later version of the list is the only signal the source gives that an exclusion has ended. The state stamps each release with a monthly effective date.
