About this list
The Arkansas Department of Human Services publishes this list through a public search on its own portal. At the August 2026 verification it held 2,209 entries, 2,117 individuals and 92 organizations.
The scope note here is the most important thing on this page, and it is the state's own: this list covers parties excluded from doing business with any division of Arkansas DHS, not Medicaid alone. Arkansas says plainly that a person or provider can appear on it and still not be an excluded Medicaid provider. Twenty division codes appear across the entries, including the Division of Medical Services, child care and early childhood divisions, behavioral health, aging, and records sourced from federal action. Read a match here as a DHS exclusion and check the division before drawing any conclusion about Medicaid.
Each entry carries a name, a city, a state, a zip code, and the division that acted. A second column holds either the excluded organization or, for many individuals, the employer or job title recorded alongside them. Arkansas publishes no exclusion date, no reinstatement date and no term on this list, so an entry leaves it only by being dropped from a later publication. There are no dates of birth, no national provider identifiers and no license numbers, which means a candidate match rests on name and location and needs verification against the state before you act on it. The state says results may change daily.
