Published by the California Department of Health Care Services (DHCS) (monthly). The largest state exclusion list we carry: providers suspended or ineligible for Medi-Cal participation. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list
The Suspended and Ineligible list, also called the S&I List, names parties the Department of Health Care Services has suspended from Medi-Cal or found ineligible to participate. It covers both individuals and businesses, with individuals making up the large majority. The June 2026 file held more than 23,000 rows. Every row carries a date of suspension. An active period field reads 'Indefinitely effective' on every row but one, and that one is blank. The file does not state the reason for any suspension.
For identification the file publishes names in separate columns, a column of alternate names (AKA and DBA), addresses, provider type, license numbers, and provider numbers. There is no date of birth column. The file also has no separate NPI column: where NPIs appear, they sit inside the provider number field, on roughly 14 percent of rows.
With no dates of birth in the file, identification rests on names, license numbers, and provider numbers. Where a row carries a license number or an NPI, a reviewer has something firm to confirm. Any candidate match from this list is a potential match requiring verification, and a candidate that rests on a name alone calls for extra care before anyone treats the record as the same party.
DHCS releases the file monthly as a CSV, a plain text format, through the California Health and Human Services open data portal. The file has no reinstatement column. A party that comes off the list is simply absent from the next monthly file, and that absence is the only removal signal the source publishes.
