Terminated Provider List (terminations for cause)

Published by the Colorado Department of Health Care Policy and Financing (HCPF), Program Integrity Section (monthly to bimonthly). Colorado Medicaid terminations for cause, plus a companion reinstated-providers list. This is a termination list, not an exclusion list, and each record keeps the termination authority the state publishes. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list

The Colorado Department of Health Care Policy and Financing, Program Integrity Section, publishes a Terminated Provider List of providers terminated for cause from Health First Colorado (Colorado Medicaid), plus a companion Reinstated Provider List. This is a termination list, not an exclusion list. A listed provider may not receive Medicaid reimbursement. At the August 2026 verification the two files together held name, doing-business-as, national provider identifier, termination authority, and termination effective date, with an explicit reinstatement date on the companion list.

Each entry carries a single provider-name cell (older rows are last name, then first name; newer rows are natural order), an optional doing-business-as name, and a national provider identifier when one is present. The termination authority is the regulation the state cites. A candidate match from this list is a potential match requiring verification, and given the state's own framing it is a starting point for a question about a termination, never an exclusion determination.

Colorado removes reinstated providers from the termination list and publishes them on the companion list with a reinstatement effective date. A provider can appear on both lists at once when a later termination follows an earlier reinstatement. There is no published update schedule; filenames carry an effective-date stamp. A clean result against this list is not a finding that no Colorado termination exists.
