About this list

The Florida Agency for Health Care Administration publishes Medicaid Sanctioned Providers as final orders. AHCA defines a suspension as a one-year preclusion from any action that results in a Medicaid claim, and a termination as a twenty-year preclusion. Corrective action plans and fines are monetary or compliance sanctions, not participation exclusions, and are not screened as exclusions.

Each published suspension or termination carries a provider name, a Medicaid provider number, a license number and national provider identifier when AHCA has them, a provider type, a sanction date, an AHCA case number, and a violation code. Florida publishes no date of birth. A candidate match here rests on name comparison, supported by an NPI or license number where one is present, and needs verification against the state.

Florida statute requires AHCA to update the list at least monthly. The file is an append-only log of final orders: an expired one-year suspension can still appear. There is no reinstatement column. A party leaves the screened set only by disappearing from a later export. A clean result against this list is not a finding that no Florida suspension or termination exists.
