About this list

The Georgia list names both individuals and business entities, and a single row can carry a person, a business, or both.

The spreadsheet publishes last, first, and middle names for individuals, a business name for entities, a state, a sanction date, and an NPI column. It does not include dates of birth. Where a row carries no NPI, a candidate match rests on name comparison alone and needs closer verification from the reviewer.
