Published by the Hawaii Department of Human Services, Med-QUEST Division (irregularly). One combined list of exclusions and reinstatements: a listed provider may carry a reinstatement date, and most rows read Indefinite. Appearing on the list is not by itself proof of a current exclusion. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.
