Each entry is a single name cell. People are written last name, then first name. Organizations have no comma. Some rows carry an also-known-as name in parentheses. A candidate match here rests on name comparison and needs verification against the state. Additional information is the state's own reason text and is stored in those words.

The cover says the list is not updated on a regular schedule, and that updates occur only when a new exclusion is added or a provider is reinstated. Rows stay listed after reinstatement, with the reinstated date recorded on the row. If no reinstatement date is listed, the state says the provider is still excluded. A clean result against this list is not a finding that no Idaho exclusion exists.
