HFS-OIG sanctions currently in force, covering terminations, bars, suspensions, exclusions, and voluntary withdrawals. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list
Illinois publishes this list as its Active Sanctions view, a snapshot of the actions in force at the time of the export. It is not a cumulative history of past actions. Five action types appear: termination is by far the most common, followed by bars, voluntary withdrawals, exclusions, and suspensions. Individuals and organizations appear together in one provider name column. Action dates on current rows reach back to 1979, so an old action that remains in force stays listed.
The export carries twelve columns: provider name, license number, NPI, provider type, affiliation, action date, action type, and address details. Field coverage is uneven. At our verification most rows carried an address, fewer than half carried a license number, and roughly one row in five carried an NPI. There is no date of birth column. Candidate matches from Illinois therefore resolve on name, strengthened by an NPI or license number when one is present, and they warrant careful review before anyone concludes that a listed party and a specific person are the same.
The Illinois Department of Healthcare and Family Services Office of Inspector General serves the list through its sanctions portal, which generates the Excel export at the time of each request. The state does not post a fixed download file. The list has no reinstatement or end-date column. When a sanction ends, its row leaves the next export, and that disappearance is the removal signal the source provides. The office's own advisory on the list is to take no action on a provider until the status has been verified with HFS-OIG, which matches how screening results should be read: as potential matches requiring verification.
