Indiana's Family and Social Services Administration publishes this record of providers whose participation in Medicaid and CHIP was terminated for cause, through its Office of Medicaid Policy and Planning. At the August 2026 verification it held 112 entries, 75 individuals and 37 businesses. The state describes the conduct behind these terminations as including adverse licensure actions, federal exclusion, fraudulent conduct, abuse or misuse of billing privileges, and falsified enrollment information or medical records, and it notes that list is not meant to be exhaustive.

A national provider identifier is present on 105 of the 112 entries. No date of birth and no license number are published. A candidate match from this list is a potential match requiring verification, and given the state's own framing it is a starting point for a question, never a current-status claim about the party named.
