Iowa Medicaid Sanctions List
Published by the Iowa Department of Health and Human Services, Program Integrity (irregularly). Iowa Medicaid terminations, participation suspensions and re-published federal exclusions. Roughly a quarter of the list is suspension for overpayment, which is a debt status rather than an exclusion, so each record carries its sanction type. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list
Iowa Health and Human Services publishes this list through its Program Integrity unit. At the August 2026 verification it held 1,307 entries, 1,053 individuals and 254 organizations.
Iowa records five kinds of action and the difference matters when you read a result. Terminations account for 475 entries and federal exclusions for 225. Participation suspensions account for 237. A further 369 entries, more than a quarter of the list, are suspensions for overpayment, which describes a provider who owes money rather than one who has been excluded. Every entry carries its own sanction wording exactly as Iowa published it, so you can see which kind you are looking at rather than having to assume.
Each entry carries a last and first name or a legal business name, an enrollment type, a specialty, an effective date, and the authority Iowa cites for the action. A national provider identifier appears on about 400 entries. Others carry an Iowa provider number instead, and 552 entries carry no identifier at all, so many candidate matches rest on name comparison and need further verification. Some entries carry an end date or a date the provider may reapply. Those are not reinstatements, and Iowa continues to publish entries whose end date has already passed, so this product does not treat them as clearing anyone.
