Published by the Kansas Department of Health and Environment, Division of Health Care Finance (as needed). Kansas Medicaid providers whose participation was terminated. This is a termination list, not an exclusion list, and Kansas sends readers to the federal LEIE and SAM for exclusions. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list

The Kansas Department of Health and Environment, Division of Health Care Finance, publishes this Terminated Provider List. It is a termination list, not an exclusion list. Kansas says so on the same page, and it sends readers to the federal LEIE and SAM for people excluded from reimbursement, including people who never appear here. At the August 2026 verification it held 206 entries: 172 individuals and 34 organizations. Ten entries also carry a doing-business-as name, so those are screened as both the named party and the business.

Each entry carries a termination date, a name, a provider type, a Kansas Medical Assistance Program identifier when one is present, a national provider identifier when one is present, and a comment in the state's own words. One hundred fourteen entries carried at least one real NPI: 109 were a single bare number, four listed more than one, and one paired a real NPI with the marker N/A. Ninety-two used the literal marker N/A. Kansas publishes no date of birth and no professional license number. A candidate match from this list is a potential match requiring verification, and given the state's own framing it is a starting point for a question about a termination, never an exclusion determination.

The page states the list is not updated on a schedule, but is updated as needed, and the workbook at verification was stamped REVISED: August 21, 2026. There is no reinstatement or end-date column. A provider leaves the list only by being dropped from a later publication. A clean result against this list is not a finding that a provider was never terminated.
