Published by the Louisiana Department of Health (on a rolling basis). LDH adverse actions including exclusions and terminations, one of the few state lists publishing dates of birth. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list
Louisiana's file is wider in scope than a single exclusion register. It records several kinds of adverse actions: state and permissive exclusions, for-cause terminations, federal OIG exclusion actions that the state mirrors from the LEIE, and registry findings against certified nurse aides and direct service workers. Both individuals and businesses appear on it. Because federal actions are mirrored, a party listed on the federal LEIE can appear here as well. Seeing the same action in both places is expected.
Rows carry names, an NPI column, an affiliated entity, the program office, an effective date, and a free-text reason for the action. A date of birth appears on roughly 71 percent of rows, which is rare among state exclusion lists. For review, a published birthdate gives the reviewer a second fact to check a candidate match against, so Louisiana candidates can be confirmed or ruled out with more confidence than name-only candidates from lists that publish no birthdates.
The Louisiana Department of Health serves the file from its adverse actions search portal. The download is a comma separated text file, titled the Louisiana State Exclusion List, generated at the time of each request. The state does not post a fixed dated file. The export includes a reinstatement column: when the department ends an action, the reinstatement date appears there. Rows that have not been reinstated carry a far-future placeholder date in that column.
