About this list
The Office of MaineCare Services, Program Integrity, publishes this Monthly Provider Exclusion Report from a public library on the MaineCare portal. The state calls it an exclusion report. A new folder appears at the start of each calendar month. The name-search tool on the same portal is a lookup, not the file this page describes.
Each entry carries a last name and first name, or an organization name in the last-name cell, an optional middle initial, up to four alias name pairs, a provider type, and a state exclusion start date. Maine publishes no national provider identifier, date of birth, or license number on the downloadable report. A candidate match here rests on name comparison, including the aliases the state published, and needs verification against the state.
The office says the report is updated the first of each calendar month and may not include a provider added later that same month. There is no reinstatement or end-date column. A party leaves the list only by being dropped from a later publication. A clean result against this list is not a finding that no MaineCare exclusion exists.
