Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. About this list
Maryland lists organizations and individual providers together on one sheet. Rows carry a sanction type code, and the file ends with a short legend defining the codes, which include MDH, HHS, LB, F, and MFVVU entries. The categories are broader than Medicaid exclusion alone: LB rows, for example, record licensing board discipline. Reading the code on a row tells a reviewer what kind of action the row records.
Published fields are the last name or organization name, first name, type of entity or profession, sanction type, NPI, license number, termination or sanction date, and address. A row with no first name records an organization. Provider chains can appear on several rows with the same name and date, one per address. There is no date of birth column, so a candidate match from Maryland rests on the name plus whatever corroborating fields line up, such as an NPI or a license number when present. Name-only candidates from this list need closer verification before a reviewer treats the listed party and a roster member as the same person.
The Maryland Department of Health publishes the list as one Excel spreadsheet and overwrites the same file in place when it updates. The state does not post dated back-issues. The file carries its own as-of date at the bottom, next to the code legend. There is no reinstatement or removal column. A provider who comes off the list is simply absent from the next posted copy, so removals are visible only by comparing one version of the file with the next.
