Michigan Sanctioned Provider List

Published by the Michigan Department of Health and Human Services (MDHHS) (on a rolling basis). MDHHS sanctions covering entities and individuals, including dual rows naming both. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list

Every row on the Michigan list is a sanction currently in force. When a sanction ends, MDHHS removes the row from the file, so the list is a snapshot of active sanctions and does not keep a history of ended ones. It covers organizations and individuals. At our verification in August 2026 the file held about 4,000 rows: roughly 3,500 named a person only, about 280 named an entity only, and another 278 named both an agency and a principal person on the same row.

Each row can carry the sanction date, the sanctioning authority, and a reason, and a single row sometimes lists more than one date and more than one authority. The reason field is blank on about six in ten rows. Where a reason is present, wording such as 'Summary Suspension Terminated' describes an enrollment termination event; the row remains an active sanction for as long as it stays on the list.

For identification the file publishes entity name, person names in separate last, first, and middle columns, provider category, NPI, city, and license number. It does not publish dates of birth. A row with an NPI gives the reviewer a strong identifier to confirm against. Candidate matches from this list are potential matches requiring verification, and a match that rests on a name alone calls for extra care before anyone concludes the record names the same party.

MDHHS publishes the list as an Excel workbook with one data sheet, updating it on a rolling basis, typically one to three times in a month. The top row of the sheet is a title banner that carries the list's as-of date. The file has no reinstatement column; removal from a later edition is how the state signals that a sanction has ended.

Our copy's freshness

Every pull is archived with a SHA-256 checksum before parsing, and anomalous changes are quarantined automatically while the last verified version keeps serving. Live state for all sources is on the status board.
