About this list

The Minnesota Department of Human Services publishes two Minnesota Health Care Programs Excluded Provider Lists from its provider manual: excluded individual providers and excluded group providers. MHCP says it updates both lists monthly. A listed party is excluded from receiving payment from Medicaid funds. The lists also include providers terminated for fraud, theft, abuse, error, or noncompliance.

Each individual entry carries a last name, first name, optional middle name, provider type, exclusion status, an effective date, and an address when one is present. Each group entry carries a practice name, provider type, exclusion status, an effective date, and an address. Minnesota publishes no national provider identifier, date of birth, or license number. Exclusion status is an authority flavor (Medicaid authority, Medicare Medicaid exclusion, or Denied not eligible), not an open or closed flag. A candidate match here rests on name comparison and needs verification against the state.

There is no reinstatement or end-date column. A party leaves the list only by being dropped from a later publication, after a written request to DHS Program Integrity Oversight. A clean result against these lists is not a finding that no Minnesota exclusion exists.
