About this list
The Mississippi Division of Medicaid publishes this list from its provider terminations page, citing Mississippi Code 43-13-121 and the federal regulation at 42 CFR 455.416. At the August 2026 verification it held 218 entries, 145 individuals and 73 organizations.
Mississippi records the basis for each action in its own sanction coding, which includes actions taken by the state, actions taken by a licensing board, and actions that follow a federal exclusion or a Medicare exclusion. Some of these entries therefore restate an action that also appears on the federal lists. Where an organization is listed and a named person is recorded as its provider or owner, both are screened, because the person is a sanctioned party in their own right and screening only the organization would miss them.
Each entry carries separate name fields for people and organizations, an address, a provider type and specialty, a termination effective date, and the reason in the state's own words. A national provider identifier appears on 164 entries and a date of birth on 99, which gives those candidate matches more to compare than a name alone. Most entries are open ended: Mississippi writes an end date far in the future to mean indefinite, and this product does not read that as a scheduled reinstatement. The state tells providers to check the list monthly.
