About this list
Missouri Medicaid Audit and Compliance, part of the Department of Social Services, publishes this list of providers terminated from Missouri Medicaid under state regulation 13 CSR 70-3.030. At the August 2026 verification it held 279 entries, 175 organizations and 104 individuals. The list reaches back to terminations from February 2011 onward.
One scope note matters for how a clean result should be read, and it is the state's own: not every provider terminated since 2011 appears here. Missouri lists only those it deemed potentially detrimental to the program. No result against this list, or any other, establishes that a provider was never terminated.
Each entry carries a provider name, a termination effective date, a provider type and specialty, and the reason for the termination in the state's own words. A national provider identifier is present on 278 of the 279 entries and a published license number on 78. The same provider can appear on more than one entry when the termination covered more than one provider type. Missouri publishes no addresses and no dates of birth. The state reviews the list monthly and has no reinstatement column, so a provider leaves it by being dropped from a later monthly file.
