About this list
The New Hampshire Department of Health and Human Services publishes this Provider Exclusion and Sanctions List from its Medicaid provider relations page. The state uses both words. At the August 2026 verification it held 76 entries: 63 individuals and 13 organizations. Three individual rows also name a business, so those entries are screened as both the person and the business. Nine entries are typed as Medicaid recipients rather than enrolled providers, and they remain on the published list.
Each entry carries a last name and first name, or a business name, a provider type, a national provider identifier when one is present, the reason in the state's own words, and an effective date. Reasons mix board actions, federal Office of Inspector General exclusions, Medicare revocations, and Medicaid fraud convictions. Fifty-one of the 76 entries carried a real NPI. Twenty used the literal marker n/a, four were blank, and one cell held a federal employer identification number rather than an NPI. New Hampshire publishes no date of birth, no license number, and no address. A candidate match here rests on name comparison, supported by an NPI where one is present, and needs more verification from the reviewer.
The workbook banner at verification read Updated 5/21/26. There is no published schedule and no reinstatement or end-date column. A party leaves the list only by being dropped from a later publication. A clean result against this list is not a finding that no New Hampshire sanction exists.
