About this list
North Dakota Health and Human Services publishes this list from its Medicaid fraud and abuse page. At the August 2026 verification it held 193 entries, 184 individuals and 9 organizations, and the file itself was stamped as current to 13 July 2026.
The list combines state terminations for cause with entries whose stated reason is a matching federal exclusion, and it keeps both the sanction type and the reason in North Dakota's own words, so you can tell which you are looking at. Around 76 entries cite a federal exclusion, so a share of this list restates action you will also find on the federal lists.
Each entry carries a name, an address, a provider type, the exclusion date and the reason. A national provider identifier appears on 47 entries. That number is low by design rather than by omission: North Dakota states that qualified service providers, who make up more than half the list, are never issued one, so those candidate matches rest on name comparison and need verification against the state. Seven entries carry an alternate name, which is screened alongside the primary one. A second column on some entries records an affiliated business rather than a second excluded party, and this product never treats it as one. There is no reinstatement column.
