About this list

The Oregon Health Authority names providers prohibited from serving Oregon Health Plan members in a sanctions list kept on its provider enrollment page. It is the smallest list we carry: three entries at the August 2026 verification. A short list is not an incomplete one. States differ in what they publish, and Oregon maintains this one as a running record rather than a cumulative history.

Each entry carries a provider name, a national provider identifier, the state's provider type classification, an effective date, and the duration of the sanction, which is currently indefinite on every entry. No date of birth is published, so a candidate match rests on name and identifier comparison and needs more verification from the reviewer.

The authority maintains the list in place rather than publishing a dated file each period, and entries change as sanctions occur. There is no reinstatement column: a provider leaving the list is the only signal the source gives that a sanction has ended.

Our copy's freshness

Every pull is archived with a SHA-256 checksum before parsing, and anomalous changes are quarantined automatically while the last verified version keeps serving. Live state for all sources is on the status board.
