About this list
The South Carolina Department of Health and Human Services compiles this list through its Bureau of Internal Audit and Program Integrity. At the August 2026 verification it held 1,349 entries, roughly 200 of which name an organization rather than a person.
The list mixes two different findings and keeps them labelled. 1,034 entries are exclusions and 315 are terminations for cause. South Carolina also states that it compiles parties excluded by the federal government as well as by the state, so a share of these entries restates an action you will also find on the federal lists. Each entry keeps the state's own wording for which finding applies.
Each entry carries a single name field covering both people and organizations, a city, state and zip code, the last known profession or provider type, and the date of the action. A national provider identifier appears on about 520 entries; the rest record that none was found, so those candidate matches rest on name comparison and need verification. There is no reinstatement column, so a party leaves the list by being dropped from a later publication. South Carolina states no publication schedule, and the two publications observed were about two months apart.
