Published by the TennCare Office of Program Integrity (as needed). TennCare's list of providers terminated for cause. This is a termination list, not an exclusion list, and each record keeps the state's own reason. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list

TennCare's Office of Program Integrity publishes this Terminated Provider List. It is a termination list, not an exclusion list. At the August 2026 verification TennCare published 21 entries: 15 individuals and 6 organizations. One organization row also names a person after a doing-business-as marker, so that entry is screened as both the business and the person.

Each entry carries a last name, a first name, a national provider identifier when one is present, an effective date, and the reason for the termination in TennCare's own words. The reason is stored exactly as published. Twelve of the 21 entries used the literal marker N/A in the identifier column rather than an NPI, so those candidate matches rest on name comparison and need verification against the state. Organizations appear with the whole name in Last Name and an empty First Name. TennCare publishes no date of birth, no license number, and no address.

TennCare states that the list is current and is updated as needed. There is no published schedule and no reinstatement or end-date column. A provider leaves the list only by being dropped from a later publication. A clean result against this list is not a finding that a provider was never terminated, and it is not an exclusion determination.
