About this list

The Department of Vermont Health Access publishes this list through the Vermont Medicaid provider portal. At the August 2026 verification it held 67 entries, 54 individuals and 13 organizations, published as a monthly list of for cause exclusions.

Vermont states that this list may not be considered comprehensive of all providers excluded from participation in Vermont Medicaid, because action by other federal or state bodies can also result in exclusion. A clean result against it is not an assurance that no exclusion exists, which is why this product screens both federal lists for every roster regardless of state.

Each entry carries a Vermont Medicaid provider identifier, a full name, a provider type, a city and state, the reason for the exclusion in Vermont's own words, and an effective date. A national provider identifier appears on 51 of the 67 entries, which gives most candidate matches more to compare than a name alone. Entries are open ended: Vermont writes an end date far in the future to mean indefinite, and this product does not read that as a scheduled reinstatement. A separate reinstatement column exists and is empty on every current entry.
