Washington Provider Termination and Exclusion List

Published by the Washington State Health Care Authority, with a companion listing from the Department of Social and Health Services (irregularly). Providers terminated for cause or excluded from Washington Apple Health, published alongside a social services listing. Healthcare organizations screen staff and vendors against it to protect their program participation and billing eligibility. This page shows aggregate statistics only; it never lists or searches individual records.

About this list

Washington publishes two listings together: providers the Health Care Authority terminated for cause or excluded from Apple Health, the state's Medicaid program, and a companion listing of social services providers from the Department of Social and Health Services. At the August 2026 verification the two held 235 published rows between them, which we screen as 246 separate identities, because a single row sometimes names both a practitioner and the clinic involved. That worked out to 229 individuals and 17 organizations.

Each Health Care Authority entry carries a name, an exclusion date, and the action the state took, with a license number on almost every row. The single social services entry carries no action field. Identifiers are mixed: a row may carry a national provider identifier or a Washington ProviderOne number instead, and a validated NPI is present on about three quarters of the identities we screen. Neither listing publishes a date of birth, so a candidate match rests on name comparison supported by an identifier where one is present, and needs more verification from the reviewer.

Washington states that providers terminated without cause are not on these lists, and it cautions that not every action taken by either agency involves a finding of wrongdoing. Read an entry as an action the state reported, not as a conclusion about the person or organization named. The lists carry no reinstatement column: a provider who returns to good standing simply leaves a later revision, and that removal is the only signal the source provides. Updates are irregular, and each revision is dated.

Every pull is archived with a SHA-256 checksum before parsing, and anomalous changes are quarantined automatically while the last verified version keeps serving. Live state for all sources is on the status board.
